Admission Criteria
Medicare has set forth the following criteria for home health admissions:
- Patients must have physicians orders.
- Patients must be homebound:
- the patient may leave home infrequently and only with considerable and taxing effort;
- the absences from home are primarily attributable to the need to receive medical treatment.
- Patient must require the intermittent services of a skilled nurse and/or a physical therapist.
The home health service is a necessary and reasonable treatment of illness or injury, which cannot be performed by someone other than a skilled health care professional.
Private Pay Services
Our goal is to help you maintain your highest level of independence through our in-home services. These services are available 4-24 hours daily.
During your initial visit our coordinator will perform an assessment of your environment, lifestyle, and health status to create an individualized plan
to meet your needs. We work together with you, your family, and anyone else you designate to assist in meeting your needs.
Caregivers are carefully screened and
have a thorough criminal background check.
Employees are bonded and insured. For many
people, remaining at home means remaining
independent. Our caring, conscientious staff
can help make that possible.
